do your proxy.org was created by Anne Fugle and further developed by Brock Roller, Ben Petre, Todd Liu, Nick Braus and Mike Nabozny, all fourth year medical students at the University of Rochester School of Medicine and Dentistry, as a project for the Community Health Improvement Clerkship, a four week clerkship that allows students to design and execute sustainable projects to improve community health.  Credit must be given to Jason Chang, MD a neurology resident at Strong Memorial Hospital, whose idea inspired this project, and both Timothy Quill, MD, director of the Strong Health Palliative Care Program and Jane Greenlaw, JD, Director of the Division of Medical Humanities and co-director of the Palliative Care and Clinical Ethics Program at the University of Rochester School of Medicine and Dentistry, who served as advisors.

Thinking about end-of-life issues can be very difficult, filling out the forms should be easy. Our ultimate goal is to increase the number of people in New York State (and the rest of the country) who are thinking about end-of-life issues and to make it easier for people to legally document their wishes.
